Each resident
must have an initial wellness
assessment completed prior to
his/her move-in date. No
services other than hospitality
services for independent living
may be provided without a
wellness assessment and
evaluation completed.The
initial wellness assessment
should include:
a. History
b. Acute or unstable conditions
c. Current medications and
treatments
d. Treatments and therapies
e. Physical ability assessment
f. Functional ability assessment
The assessment shall be
updated prior to service plan
review, or whenever appropriate.
Once the wellness assessment
is completed, and reviewed by
our staff, a joint meeting will
be held with the resident and
his/her family to establish the
appropriate level of services.
Resident health status will
be re-assessed when a
significant change has occurred
or when requested by the
Administrator.
Information gathered from the
wellness assessment and
physician's orders is used to
determine whether
self-administration of
medication is appropriate.
Licensed staff provides
health information to the
Administrator to assist with the
development of a service plan
for each resident.
